Next we critically analyse the competing demands within organisations between accountability and
improvement, with particular consideration given to the role of the wider community and their perception of these approaches.

We then consider ideas of human error in a world of complexity. We explore systems based
perspectives, the dynamics within the system and how they influence our
understanding of work as well as the role of adaptive capacity.
